Visit http://forms.form-provider.com (logon with user: testadmin and password: pass) and follow the steps to create a new eForm.

Steps to Create a New eForm

  1. Visit http://forms.form-provider.com and log in using the credentials: user – testadmin, password – pass.
  2. Click on the “Create new Form” button to initiate the creation of a new eForm.
  3. Enter a unique Document ID into the form field labeled “Document ID”.
  4. Provide your email address as the notification address for form events.
  5. Describe your form in the “Document Description” field.
  6. Click on the “Create Field” button to add your PDF Objects.
  7. After creating the objects, click on them to select and then click on the drop-down list under the button “Field Properties” to define the object type.
  8. Click again on your objects to select them and then click on “Field Properties” to define text, values, images, etc.
  9. To render and view the PDF, press the “Show PDF” button. You can download the eForm with “Download PDF” on the bottom left. Please note that this eForm will work only in Adobe Reader XI or higher.
  10. If you want to release the eForm to our online form filler, press the “Release PDF” button on the very left bottom of the page. You will be notified by mail when the eForm has been rendered and released.
